We are pleased that you are interested in participating in one of the 2009 Livingston CARES work trips to Biloxi, Mississippi. You will be continuing the Hurricane Katrina reconstruction work that over 300 volunteers started during our eleven previous trips to Harrison County, Mississippi. This application packet includes the documents required by the parties involved in making these trips possible. The first three forms are required by Livingston CARES. The Camp Victor Ministries run by the Lutheran-Episcopal Services in Mississippi (LESM) in Ocean Springs, Mississippi will be providing our accommodations and meals. Volunteers will be working at sites in East Biloxi run by The East Biloxi Coordination, Relief & Redevelopment Agency (EBCRRA). Each party has its own separate legal documents that are required in order to provide the service to trip participants.

Instructions: You will need to complete each of the forms.  The Livingston CARES Release and Waiver of Liability Form is the only one that requires the witness of a Notary Public. Notarization of your document may be completed by the staff in the Federal Credit Union located just outside the MacVittie mail room.  The other forms may be signed by any witness over 18 years of age. 

You must photocopy your Medical Insurance ID Card and the front of your driver’s license or another photo ID and attach to the front page of your Medical Information Form.

Deadlines: The deadline is October 24 for completed applications for the January, February and March trips. Paper work and checks must be returned to Tom Matthews, Kay Fly or Elise Arneson in the Center for Community in MacVittie College Union Room 353. Seats will be filled on all trips on a first-come, first-serve basis although twenty percent of the seats will be held for new participants until October 24. The deadline is March 13 for completed applications for the May trip.

Payment: You will need to make a check payable to Livingston CARES for the full amount for the January trip ($400.00 for SUNY Geneseo students and $500.00 for non-students), or a deposit of $100.00 for the March trip with the balance due by February 1.

Participants in the February trip will receive separate payment instructions. Please note that all trip expenses are tax deductible under IRS regulations. Limited financial assistance is available by picking up an application from Elise Arneson, Kay Fly, or Tom Matthews in Room 353 in the College Union.

All trip participants will also be required to attend a training session that will be conducted by Livingston CARES in late November or early December.
